Important Tip:<\/p>\n
Please ensure that the order\/spelling of the name and date of birth on the proof of address matches the information on the ID card. If they do not match, send a statement\/affidavit (original) stating the name and date of birth on the ID card and the name and date of birth on the proof of address with the correct spelling and stating that both names and dates of birth belong to the same person<\/p>\n
The signature on the power of attorney should be the same as the signature on the individual's identity card\/passport\/driving licence<\/p>\n
Some information may be obtained directly from sources such as MCA, NSDL, or through mechanisms such as penny tests, in which case you may not need to upload some of these documents<\/p>\n
